This form is intended for individuals and organizations that wish to publicly endorse the letter. Please complete the information below exactly as you would like it to appear in the published version.

If you are signing on behalf of an organization, you will have the option to upload a high-resolution version of your organization's logo (PNG or JPEG preferred). Submitting a logo is not a requirement to sign-onto the document. Logos may be displayed alongside your organization's name in published materials.
